1. Complete the RAFOS Permission Form. (Due September 12th)

• You can download a copy of this using this link.
• The permission form also has a driving arrangement and volunteer interest form that should be filled out. It has questions more specific than the standard RAFOS form that we need to make travel arrangements.

2. Complete the Sly Park Environmental School Registration form on their website. (Due September 12th)

• Use the code RAG to register as a Gateway student for the GHR ID .
• You will need to create an account.
• You do not pay for the trip here, but you should purchase and merch you’d like your child to have.

3. Fill Out Transportation Preferences Form

4. Complete an Overnight Medication Form (Due September 26th) - This is optional, only for is meds are being brought

• You can download a copy of this form above.
• If you are sending your child with ANY medication, OR you want teachers to be able to administer over-the-counter medications (Tylenol, Antacid, Cough Drops, etc.) then you must have this form completed and signed by a doctor.

<aside> ⚠️ When registering on the Sly Park website, you may be asked to give permission for over-the-counter medications. Please ensure you have completed the RAFOS medication documentation, as this information does not allow you bring any medications.

5. Chaperone Volunteer Documents (For those who want to be chaperones)

• Complete the interest form that was sent out by your teachers.
• Volunteering paperwork needs to be submitted by Friday, September 19th, and includes:
  • TB Clearance & Live Scan Clearance (Live Scan must be completed for RAFOS School specifically)
  • Volunteer Agreement (Each year requires a new agreement)
